Community Health Grant Funds RFP
The FY 2011 State budget passed by the General Assembly contained funding for the Community Health Center Grant Program. The purpose of grants awarded under this program is to increase access to primary care for uninsured and medically indigent patients in the state. As a non-profit primary care safety-net organization, you are eligible to apply for a grant to fund a project targeted at increasing access to primary medical and preventive care. Grants will be awarded for projects/initiatives that do one of the following: (a) increase access to primary medical care for uninsured and medically indigent patients at existing facilities; (b) develop a new primary medical care service delivery site; or (c) create or expand primary medical and preventive services provided to uninsured and medically indigent patients.
They anticipate being able to award 7 to 9 new program grants this year; this will be an extremely competitive cycle. Only one program grant application per county or service area will be funded. The maximum total grant award is $125,000 for solo-organization grants and $175,000 for joint-organization grants. Separate capital grants will not be funded during this grant cycle. However, applicants may include requests for capital directly related to their proposed project in their program grant application.
The FY 2011 State budget funding also may be used to expand the Office of Rural Health and Community Care’s Medical Access Plan (MAP). The MAP Program provides grant funding tied to the direct provision of indigent care by a non-profit primary care medical provider. To confirm that you are eligible to apply for a MAP grant, please contact Parcheul Harris at the Office of Rural Health and Community Care BEFORE you submit an application. To apply, please complete the application and submit one original and four copies (five grant applications total), along with one copy of your most recent audit and IRS letter verifying your tax exempt status, to Dr. Andrea Radford at the Office of Rural Health and Community Care. All applications must be either postmarked by Wednesday, September 29, 2010 OR received by 5:00 pm Monday, October 4, 2010. Electronic copies (email or fax) will not be accepted.
